First Time to Our Office?
WELCOME! Your first visit to our office will consist of a complete oral examination. We'll also ask that you take a moment to fill out several general information forms or you may download them below and fill them out prior to your arrival.
We will take any necessary x-rays and perform other tests that may be needed to make a complete diagnosis of the condition of your mouth, teeth and gums. We will then be able to determine the treatment that may be required.
